Sambisari Temple is located at Sambisari village, Purwomartani, Kalasan. This temple was found accidentally by a farmer working on his land in 1966. This temple had been covered by the cold lava from Mount Merapi more or less 6,5 meter.

The division of the temple mostly similar to Prambanan Temple, but in a simple shape, since Sambisari Temple is a watak temple and Prambanan Temple is a kingdom temple.

The area of Sambisari Temple consists of a main temple and three perwara temples on the front side. The first area covers 50 x 48 meters, sorrounded by white-stoned fence. The main temple sizes 13,65 x 13,65 meters and 7,5 meters in height, from foundation (foot), body, and top. It faces to the west and has only one room (bilik). The three perwara leaves only the foot part without thebody and the top; each 4,80 x 4,80 meters.

Inside the bilik of the main temple, there is lingga-yoni with cerat yoni facing to north. Bilik also has several relung containing arca. The north relung has Durga, the east relung has arca of Ganeca, and the south relung has arca of Agastya. On the right-left side of the entrance, there is also relung without any arca.
